Location Code and Location Number - How are these elements assigned and used?

A:   

Please note that the Locator Code will no longer be supported by NYSDOH in HIPAA-regulated electronic transactions after the eMedNY implementation of the National Provider Identifier. However, atypical (non-healthcare) providers, who are not eligible to get an NPI, will still be required to use the Locator Code in their transactions to NYSDOH.

NYSDOH assigns the 3-digit Locator Code along with the NYS Medicaid Provider ID. If the provider renders service to NYS Medicaid clients at more than one location, an individual Locator Code is assigned to each location. Locator Codes range from 003 through 020. After eMedNY NPI implementation, the Locator Code will only be accepted by NYSDOH in claims for “atypical providers”, those who do not meet the Federal statutory definition of “healthcare providers”. However, Locator Code will be derived from the 9-digit zipcode, and will continue to be used internally in claims processing.

Note to Providers: To review your Locator Code(s), check the documentation you were provided with when you received your Provider ID.

Billing Provider
The following is only applicable prior to the eMedNY NPI implementation, or for claims for non-healthcare providers.

In Institutional and Professional electronic claims (X12 837 transactions), the Locator Code is sent in the Billing Provider Name loop (2010AA) in the Location Number data element in an REF segment. This is the Billing Provider Secondary Identification segment. NYSDOH requires two iterations of this segment to satisfy the business requirements for processing NYS Medicaid claims. An iteration of the REF segment must be present with the NYS Medicaid Provider ID of the billing provider, as indicated by a Qualifier of “1D” in REF01. Another iteration contains a Locator Code associated with the NYS Medicaid Provider ID, as indicated by a Qualifier of “LU” in REF01.

The Locator Code can appear in additional locations in the 837 Professional and 837 Institutional claims, in which case it can potentially override the Locator Code that was sent in the Billing Provider loop.

Rendering Provider (837 Professional)
The following is only applicable prior to the eMedNY NPI implementation, or when the Rendering Provider loop (2310B or 2420A) is sent in an 837 Professional transaction for non-healthcare services.

NYSDOH requires an iteration of the Rendering Provider Secondary Identification REF segment to send the Location Number associated with the Rendering Provider, only if the Rendering Provider was identified by a NYS Medicaid Provider ID (also sent in an iteration of the Rendering Provider Secondary Identification REF segment), and the Locator Code for the Rendering Provider is different than the Locator Code in the Billing Provider Name loop (2010AA). The Locator Code is sent in the Location Number data element in REF02, as indicated by a Qualifier of “LU” in REF01.

Service Facility Location (837 Professional)
The following is only applicable prior to the eMedNY NPI implementation, or when the Service Facility Location loop (2310D or 2420C) is sent in an 837 Professional transaction for non-healthcare services.

When the Service Facility Location loop is sent in an 837 Professional transaction, NYSDOH requires an iteration of the Service Facility Location Secondary Identification REF segment to send the Locator Code associated with the Service Facility Location only if different than the Locator Code in the Billing Provider Name loop (2010AA). The Locator Code is sent in the Location Number data element in REF02, as indicated by a Qualifier of “LU” in REF01.

Service Facility Name (837 Institutional)
The following is only applicable prior to the eMedNY NPI implementation, or when the Service Facility Name loop (2310E) is sent in an 837 Institutional transaction for non-healthcare services.

When the Service Facility Name loop is sent in an 837 Institutional transaction, NYSDOH requires an iteration of the Service Facility Name Secondary Identification REF segment to send the Locator Code associated with the Service Facility only if different than the Locator Code in the Billing Provider Name loop (2010AA). The Locator Code is sent in the Location Number data element in REF02, as indicated by a Qualifier of “LU” in REF01.

Locator Code in the 835 (Electronic Remittance Advice)
After eMedNY NPI implementation, NYSDOH will not return the Locator Code in the electronic remittance advice (835 transaction) for any claim type. Providers who have a need to receive back the Locator Code can send the Locator Code as part of the Patient Control Number (in loop 2300, data element CLM01) in the 837 electronic claim. This data element is returned in the 835 in loop 2100, data element CLP01. NYSDOH supports (will capture and return) up to 20 positions in CLM01.
