On page 16 under Pay and Seek: Claims with diagnosis ranges 671.0 - 675.0 qualify for pay and seek reimbursement if there is a fifth digit. Claims with diagnosis ranges 651.0 - 658.9 qualify for pay and seek reimbursement only if the fifth digit is a three. |
