Prior Authorization Guidelines (PDF 159KB) The Medicaid Transportation - NYC Ordering Guidelines Manual replaces the PA Guidelines Manual with more comprehensive information regarding the request for transportation of NYC Medicaid enrollees. Ordering providers are strongly urged to maintain a copy of this manual in their records.

Policy Guidelines (PDF 161KB) The definition of Advanced Life Support (ALS) ambulance service has been further refined since the last update of this manual, to conform with the Bureau of Emergency Services ALS definition.

Policy Guidelines (PDF 259KB) Changes brought about by the implementation of eMedNY have been made (i.e., Prior Authorization number is now 11 digits, not 8; description of fields on roster is changed).

The policy on the claiming of mileage within urban areas, such as New York City, is expressed.

The definitions of advanced life support (ALS) and basic life support (BLS) ambulance services have been changed, to now include the monitoring of an intravenous (IV) solution as an ALS service.

Enrollment requirements, when there is more than one operating location, have been updated.
