Potential Causes:
The required service provider identifier was not sent. This edit applies to Dental, Clinic, and Vision Care claims. For Clinic claims, which are billed on the 837 Institutional, the service provider is sent in the Attending Provider loop. For Dental (837 Dental) and Vision Care (837 Professional) the service provider is sent in the Rendering Provider loop.

Solution:
Enter the Attending Provider (837 Institutional) or Rendering Provider (837 Professional or 837 Dental) Medicaid ID or license number.
