Potential Causes:
The claim was submitted with a date of service that was over 90 days old on the receipt date of
the claim, and there was no delay indicator on the claim.

Solution:
Check the date of service to ensure that the correct date is entered. (Please enter the date using
the following loop and segment provided below.)

If the date of service is over 90 days old on the date of submission, the claim must have a 90-day
late reason code on the claim. A 90-day late reason code can only be used if the reason for the
late submission falls within the allowable reason for a late submission.
A valid delay indicator is required in the following loop and segment for the 837I, 837P, and 837D.
