Potential Causes:
The location code is missing, blank, or the provider input something they do not have on file. All claims require a location code. Valid location codes for billing purposes are 03-20.

Solution:
Verify that your location code is correct and re-submit. The location code must be entered in the second iteration of the following loop and segment for the 837I, 837P, and 837D.
