Potential Causes:
You are billing a claim with a date of service that is more than two years old.
Solution:
Check the date of service that is being billed and if it is correct then the provider may want to contact the two-year claim review unit at 1-800-342-3005, option 3 or (out of state) 518-473-4029.
If it is incorrect, correct the date of service and rebill the claim.
