Potential Causes:
More than one Value Code exists on the claim with a value of “24” (Medicaid Rate Code). Only one Medicaid Rate Code can be sent on one claim.
Solution:
Review the charges that were entered on the claim. If billing for more than one Rate Code is accurate each Rate Code must be entered on a separate claim.
