Provider Enrollment


To enroll an Assisted Living Program in New York State Medicaid, prospective providers must be licensed by the Bureau of License and Certification, Division of Home and Community Based Services, Office of Long Term Care.

Providers should contact Guy Warner, Director of the Bureau of License and Certification or Joseph J. Meloveck, Jr. in the Adult Care Facilities Unit at (518) 408-1624 for information on how to get licensed. To get information on rates contact the Bureau of Long Term Care Reimbursement at (518) 474-1057.

Enrollment Checklist
 

To expedite the enrollment process, you can begin collecting copies of documents that you will need to include when you submit the completed enrollment form:

A copy of your FEIN documentation showing both the name and number preprinted (a W-9 is not sufficient for enrollment purposes).

 

A list of the facility's or program's owners and/or Board of Directors with addresses.
