To enroll a Care at Home (CAH) Waiver Program in New York State Medicaid, prospective providers must have approval from the Office of Long Term Care, Division of Home and Community Based Waivers.
Case Management (CM) agencies must complete an application to provide CAH Case Management services. Staff from the Office of Long Term Care (OLTC) reviews the CAH provider application to ensure that the agency meets all of the qualifications. The application is put in a pending status while awaiting a determination on the Medicaid enrollment. The CMCM manual (see link in Downloads below) gives the CAH qualifications. Please Note: Existing providers have the option of billing new programs or services with an NPI already enrolled with the NY State Medicaid Program or an NPI unknown to NYS Medicaid. To report an unknown NPI, an enrollment application will be sent to you to complete and return to Medicaid's fiscal agent, Computer Sciences Corportion (CSC). Adding a new service or program to an enrolled NPI may require completion of other enrollment documents.
