To enroll a Laboratory (Hospital or Clinic-Based) in New York State Medicaid, you must possess a laboratory license from the Wadsworth Center (inside NY State) or from the appropriate home-state licensing agency (Out of State Providers). You must also submit a copy of the lab director license. Once the licensing documentation is reviewed, the facility’s representative will be contacted and an enrollment package will be mailed to the address provided. Please note that laboratory services are included in a hospital or clinic’s rate and will only be reimbursed under a Laboratory category of service in a referred ambulatory circumstance. Please Note: Existing providers have the option of billing new programs or services with an NPI already enrolled with the NY State Medicaid Program or an NPI unknown to NYS Medicaid. To report an unknown NPI, an enrollment application will be sent to you to complete and return to Medicaid's fiscal agent, Computer Sciences Corportion (CSC). Adding a new service or program to an enrolled NPI may require completion of other enrollment documents.
