To enroll a Limited License Home Care Service Agency (LLHCSA) in New York State Medicaid, you must be licensed by the Department of Health. After you are licensed, you must establish contracts with the counties in which you want to provide services.
Upstate providers must contact the County Department of Social Services. (Check the guide to local government office service listing in your phone directory to obtain a telephone number.) Agencies who wish to provide services within the 5 boroughs of New York City (Kings, Bronx, New York, Queens and Brooklyn) contact HRA (Human Resource Administration) at (212) 860-3748.
Once the New York State Medicaid Program has received notice from the NYS Bureau of Long Term Care that the appropriate contracts and approvals are in place, an enrollment package will be mailed to the Home Care Agency.
