To enroll a Long Term Home Health Care Program (LTHHCP) in New York State Medicaid, prospective providers must be certified by the Department of Health, Office of Health Systems Management (OHSM). To start the process, contact the regional Office of Health Systems Management (see the Blue State Government pages of your local phone book) or call the Bureau of Project Management @ (518) 402-0911.

Once this certification is received by the Rate Based Provider Bureau, a Medicaid enrollment package will be sent.

Please Note: Existing providers have the option of billing new programs or services with an NPI already enrolled with the NY State Medicaid Program or an NPI unknown to NYS Medicaid. To report an unknown NPI, an enrollment application will be sent to you to complete and return to Medicaid's fiscal agent, Computer Sciences Corportion (CSC). Adding a new service or program to an enrolled NPI may require completion of other enrollment documents.
