Provider Enrollment > Personal Emergency Response System (PERS)


To enroll a Personal Emergency Response System (PERS) in New York State Medicaid, prospective providers must establish contracts with the counties in which you would like to provide services.

Upstate providers must contact the County Department of Social Services. (See the Blue State Government pages of your local phone book). Agencies who wish to provide services within the 5 boroughs of New York City (Kings, Bronx, New York, Queens and Brooklyn) contact Human Resource Administration (HRA) at (212) 860-3748.

Once the New York State Medicaid Program has received notice from the NYS Bureau of Long Term Care that the appropriate contracts and approvals are in place an enrollment package will be mailed.

Enrollment Checklist
 

To expedite the enrollment process, you can begin collecting copies of documents that you will need to include when you submit the completed enrollment form:

A copy of your FEIN documentation showing both the name and number preprinted (a W-9 is not sufficient for enrollment purposes).

 

A list of the facility's or program's owners and/or Board of Directors with addresses.
