To enroll a Pharmacy (Hospital or Clinic-Based) in New York State Medicaid, you must possess a pharmacy license from the State Education Department (inside NY State) or from the appropriate home-state licensing agency (Out-of-State Providers). You must also submit a copy of the supervising pharmacist license. Once the licensing documentation is reviewed, the facility’s representative will be contacted and an enrollment package will be mailed to the address provided. Please note that pharmacy items are included in a hospital or clinic’s rate and will only be reimbursed under a Pharmacy category of service if the facility pharmacy is open to the public. Please Note: Existing providers have the option of billing new programs or services with an NPI already enrolled with the NY State Medicaid Program or an NPI unknown to NYS Medicaid. To report an unknown NPI, an enrollment application will be sent to you to complete and return to Medicaid's fiscal agent, Computer Sciences Corportion (CSC). Adding a new service or program to an enrolled NPI may require completion of other enrollment documents.
