Paper Claim Forms for Phase II : back to top
Q: Can I bill on paper if I am a hospital billing Inpatient or clinic claims?
A: No, Hospital and Clinics cannot bill on paper forms and will need to use an electronic method for billing.
 
Q: What happens if I continue submitting on the old claim forms?
A: Phase I claim forms will only be accepted until June 15, 2005. Any Phase I claim form submitted after June 15 th will be rejected and returned to the sender.
 
Q: How do I obtain the new claim forms?
A:

New claims forms can be ordered by calling Provider Services at 800-343-9000 and selecting the appropriate billing unit. Please have your Category of Service available to ensure that the correct form type is sent.

Note: Rate-based providers now use the UB-92 form which is not supplied by Computer Sciences Corporation. These forms may be obtained from any source of standardized forms.
